Social Care Coordinator for Addiction Services

The successful candidate will be responsible for delivering high-quality rehabilitation and aftercare services to individuals across the Midlands region. This will involve working closely with clients, their families and other stakeholders to provide comprehensive support and guidance throughout the recovery process.
Key Accountabilities- To develop and implement individual care plans that address the unique needs and goals of each client.
- To establish and maintain effective relationships with clients, colleagues and other stakeholders to ensure seamless service delivery.
- To contribute to the development and implementation of policies and procedures that promote best practice in rehabilitation and aftercare services.
- Familiarity with the National Drug Strategy (Reducing Harm, Supporting Recovery 2017 – 2025) and National Drug Rehabilitation Framework Document.
- A relevant 3rd level qualification (minimum Level 7 or equivalent).
- A minimum of 2 years' relevant paid employed work experience in addiction, community development, health & social care, aftercare and rehabilitation or related fields.
