Key Responsibilities:

  • Personalized Support: Provide individualized support to service users, using a facilitative approach to personal care and ensuring their right to dignity and respect at all times.

  • Assessment and Planning: Assess service users regularly through the completion of suitable assessment models and develop individual programs of support that cater to their unique needs and goals.

  • Community Inclusion: Facilitate service users' participation in their community by supporting them in using various facilities and promoting social inclusion.

  • Communication and Collaboration: Communicate effectively with family members, visitors, day services, and other professionals in a courteous and professional manner, ensuring seamless collaboration and coordination of care.

  • Record Keeping: Document and relay information to relevant parties, including multidisciplinary teams, day services, families, unit managers, and service user care plans.
