The Community Support Team at Gheel Autism Services CLG is dedicated to supporting individuals with autism to live fulfilling lives and achieve their goals. As a Social Care Worker/Autism Practitioner, you will play a vital role in promoting independence, autonomy, and self-worth, ensuring that each individual achieves meaningful outcomes. In this rewarding role, you will work closely with individuals with autism, providing person-centred care, and developing individualised plans tailored to each person's unique needs.
About the Team
Our Community Support Team operates on a support reduction model, aiming to provide the right amount of support based on individual needs. Through collaboration and ongoing review, we have achieved significant progress and reductions in support over time. As a member of our team, you will be part of a highly motivated group that shares a passion for supporting individuals with autism to reach their full potential.
Key Responsibilities
* Provide person-centred care and support to individuals with autism
* Develop and implement individualised plans tailored to each person's unique needs
* Promote independence, dignity, and autonomy
* Collaborate with the Community Support Team to achieve individualised outcomes
* Liaise and network with community services to ensure seamless support
* Experience with case management/key working and driving licensure an asset
Requirements
* Level 7 qualification in Social Care/Studies or equivalent recognised qualification
* At least one year of recent experience in the social care/disability sector
* Experience of supporting individuals with a diagnosis of ASD and knowledge of autism
* Excellent communication skills and ability to liaise with community services
* Ability to promote independence and think creatively to meet individual needs
