The primary objective of this role is to provide person-centred support to individuals with intellectual disabilities, Autism, and Acquired Brain Injury, ensuring their physical, emotional, and practical needs are met.
This will involve working closely with multidisciplinary teams, families, and key stakeholders to develop and implement individualised care plans that promote independence, dignity, and respect.
- Providing individualised support and care to residents, including personal care, meal preparation, and assistance with daily living activities.
- Acting as a key worker for assigned residents, providing guidance and support to ensure their needs are met.
- Working in partnership with MDT staff, families, and other key people to support residents to achieve their personal goals.
