Social Worker – Mental Health
Medforce are looking for a Social Worker to provide high-quality, person-centred nursing care to individuals with mental health difficulties in the community, working as part of the multidisciplinary team in Wicklow. The role focuses on assessment, care planning, treatment, and recovery support in collaboration with service users, families, and the wider health and social care system.
Key Responsibilities of Social Worker
- Conduct comprehensive mental health assessments and risk assessments.
- Develop and implement individualised care plans in collaboration with service users and the MDT.
- Deliver evidence-based interventions, including psychoeducation, medication management, and supportive counselling.
- Act as a keyworker for allocated clients and coordinate their care pathway.
- Support service users in managing symptoms, preventing relapse, and engaging with community resources.
- Participate in regular MDT meetings and contribute to case reviews.
- Promote recovery-oriented and trauma-informed care practices.
- Respond to crisis situations and participate in emergency interventions when needed.
- Maintain accurate, up-to-date clinical records in line with HSE policies.
- Liaise with primary care, social services, housing, and voluntary agencies as appropriate.
Requirements of Social Worker
- Degree in Social Work and CORU registered
- Post-registration experience in mental health (community experience preferred).
- Full, clean driving licence and access to own transport.
