We are seeking a skilled and compassionate Mental Health Social Worker to join our team.

As a key member of our multidisciplinary team, you will provide high-quality, person-centred nursing care to individuals with mental health difficulties in the community. Your primary focus will be on assessment, care planning, treatment, and recovery support in collaboration with service users, families, and the wider health and social care system.

Key Responsibilities of the Role

  • Conduct comprehensive mental health assessments and risk assessments to identify individual needs and develop effective care plans.
  • Develop and implement individualised care plans in collaboration with service users and the multidisciplinary team.
  • Deliver evidence-based interventions, including psychoeducation, medication management, and supportive counselling to promote recovery and well-being.
  • Act as a keyworker for allocated clients and coordinate their care pathway, ensuring seamless transitions and continuity of care.
  • Support service users in managing symptoms, preventing relapse, and engaging with community resources to enhance quality of life.
  • Participate in regular multidisciplinary team meetings and contribute to case reviews to ensure shared understanding and best practice.
  • Promote recovery-oriented and trauma-informed care practices, prioritising dignity, respect, and empowerment.
  • Respond to crisis situations and participate in emergency interventions when needed, demonstrating expertise and professionalism.
  • Maintain accurate, up-to-date clinical records in line with organisational policies and procedures.
  • Liaise with primary care, social services, housing, and voluntary agencies as appropriate to ensure comprehensive support and coordination.

Requirements of the Role

  • Degree in Social Work and CORU registration.
  • Post-registration experience in mental health, with preference for community experience.
  • Full, clean driving licence and access to own transport.
