Responsibilities
  • Deliver support according to the person's PCP and care plan.
  • Programme planning and use of Person Centred Plans.
  • Place the needs of the person supported as the key focus in all activities.
  • Work with people with complex needs and support their goals.
Qualifications
  • A full QQI level 5 Health Care Qualification is essential (e.g. health, community, care sector).
  • A genuine passion for providing person centred support is key to being successful in the role.
Skills and Abilities
  • Ability to work on own initiative.
  • Aware of the role and philosophy of a Key Worker/Support Worker.
  • Ability to work with people with complex needs.
  • Effective communication in fluent English.
  • Full, clean driving licence (manual) desirable.
