Post specific related information: Flexi part-time contract of a minimum of 24 hours per month.

Qualifications required: A full QQI level 5 Health Care Qualification is essential (e.g. health, community, care sector) but a genuine passion for providing person centred support is key to being successful in the role of a Support Worker.

Skills/Abilities required: Ability to work on own initiative. Awareness of Key Worker/Support Worker role and philosophy surrounding support to people with complex needs. Programme Planning and Person Centred Plans. Places the needs of the person supported as the key focus in all activities. Delivers support according to the person's PCP and care plan. Fluent English and excellent communication skills. Full, clean drivers licence (manual) desirable.

Responsibilities
  • Provide person-centred support to people with complex needs in line with PCP and care plans.
  • Plan and deliver supports in accordance with the person's PCP.

  • Work with initiative in a team setting and maintain documentation as required.
Qualifications
  • QQI Level 5 Health Care Qualification (essential).
  • Ability to work on own initiative; strong communication skills.
  • Experience or understanding of person-centred planning and supports.
  • Full, clean driver's license (manual) desirable.
