Support Worker Disability FPTP242508 role at HSE Workplace Health & Wellbeing

This is a flexi part-time contract with a minimum of 24 hours per month. Applications are invited for the role of Support Worker with a focus on disability services.

Responsibilities
  • Deliver support according to the person's Personal Care Plan (PCP) and care plan.
  • Place the needs of the person supported as the key focus in all activities.
  • Work on own initiative and be aware of the Key Worker/Support Worker role and philosophy surrounding support to people with complex needs.
  • Contribute to Programme Planning and implementation of Person Centred Plans.
  • Communicate clearly in English with the person supported, families, and team members.
Qualifications & Skills
  • Full QQI Level 5 Health Care Qualification is essential (e.g., health, community, care sector).
  • Genuine passion for providing person-centred support.
  • Fluent English and excellent communication skills.
  • Ability to work on own initiative.
  • Full, clean driver's licence (manual) desirable.
