The Community Support role involves providing direct support to individuals in their community, adhering to a client-centered approach. The successful candidate will assist people supported in achieving their goals and dreams by focusing on their desires, needs, and aspirations.
Key responsibilities include:
- Empowering individuals with decision-making autonomy.
- Fostering a supportive environment that acknowledges each person's capabilities and competencies.
- Participating in debriefing sessions after incidents and providing constructive feedback.
- Implementing individualized support plans while respecting each person's feelings and needs.
- Maintaining professionalism and dignity when interacting with colleagues, management, and people served.
- Planning community and in-home activities tailored to each person's interests and needs.
- Listening actively and following each person's lead, avoiding debate or advice during crises.
- Adjusting interactions based on verbal or non-verbal communication and assisting in deescalating situations.
- Seeking feedback from people supported, supervisory, and clinical staff to ensure alignment with agency philosophy and individual needs.
- Recognizing and acting upon non-verbal feedback provided by people supported.
- Providing transportation for community activities as needed.
- Assisting with daily living skills, including social and emotional support, communication assistance, crisis intervention, medical, and personal care.
- High school diploma or equivalent required; previous experience in the human service field is desirable.
- Valid driver's license and current vehicle insurance.
- Maintaining training compliance as defined in pertinent regulations.
- Ability to comprehend and utilize electronic mediums and various computer programs.
- Availability during scheduled licensing and monitoring.
- Effective communication and problem-solving skills.
