This role involves facilitating service users in their choices and developing programmes within a person-centred model of service delivery. The programmes may include independent living, personal development, and occupational and leisure activities, all within the context of a quality of life model.
Key Responsibilities- Develop and deliver person-centred programmes based on Person Centred Planning; maintain accurate records of progress.
- Act as a key worker for service users, facilitating goal setting and support planning; ensure that records are maintained up to date.
- Support individuals and groups in accessing community resources by facilitating activities and programmes tailored to their needs.
- QQI Level 5 in Social Care or equivalent is highly desirable.
- Experience supporting adults with intellectual disabilities and mental health issues is essential.
- Fluency in Irish is highly desirable, as is an understanding of Irish Sign Language.
- Excellent interpersonal skills and programme facilitation skills are also required.
