Job Role

We are seeking a highly skilled Care and Support Professional to join our team. The successful candidate will have a full QQI level 5 major award in Health/Care and be aware of the Key Worker/Support Worker Role and philosophy surrounding support to people with complex needs.

Key Responsibilities
  • Deliver person-centred care and support to individuals with complex needs.
  • Assess and plan individualised care plans that promote independence, dignity, and well-being.
  • Provide emotional and practical support to individuals, promoting their physical and mental health.
  • Collaborate with multidisciplinary teams to ensure seamless care delivery.
  • Develop and maintain relationships with individuals, families, and carers.
Requirements
  • A full QQI level 5 major award in Health/Care is essential.
  • Awareness of Key Worker/Support Worker Role and philosophy surrounding support to people with complex needs.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work on own initiative and as part of a team.
  • Fluent English and excellent written and verbal communication skills.
