About the Role:
- The community nurse will work within primary care in the PHN service, delivering high-quality patient care in a clinic setting and in clients' homes.
- Travel around South Dublin may be necessary for this role, with staff eligible to claim travel allowance for work-related travel.
- The service operates Monday to Friday, 9am-5pm, with negotiable hours and part-time options available.
- Onsite training and orientation will be provided by HSE staff to ensure a smooth transition into the role.

Requirements:
- Access to own transport is required for this post.
- Work authorisation is mandatory.
- NMBI registration as an RGN
- Community nursing experience is preferable.
- Acute experience working as a general nurse
- Excellent wound care experience
Responsibilities:
- Nurse-led wound care management - both in clinic and in the domiciliary setting
- Diabetes management
- Incontinence assessment and management
- Nursing assessment post hospital discharge/GP referral
- General nursing care for the chronically ill or disabled
- Oncology support and end-of-life care
- Nursing assessment for home supports
