Social Care Professional for Community Support

We are seeking a dedicated and compassionate Social Care Professional to join our team. As a key member of our community support team, you will be responsible for providing high-quality care and support to individuals with intellectual disabilities.
Responsibilities:- Develop and implement personalized care plans to meet the unique needs of each individual.
- Provide emotional support and guidance to help individuals build confidence and independence.
- Assist with daily living tasks such as personal care, meal preparation, and household management.
- Foster positive relationships with individuals, families, and other stakeholders to ensure seamless support.
- Collaborate with healthcare professionals to coordinate medical appointments and interventions.
- Bachelor's degree in Social Care or related field.
- CORU registration or equivalent qualification.
- Minimum 1 year experience working with vulnerable adults or individuals with intellectual disabilities.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
