Community Nursing Opportunity

This is an excellent opportunity for nurses to take their career in public health nursing to the next level, serving as a stepping stone to becoming a PHN. As a Community General Nurse, you will work within Primary Care in the PHN service, providing care to clients in their homes and clinic settings.

About the Role
  • Work within Primary Care in the PHN service as a community nurse, delivering high-quality patient care and developing strong relationships with clients.
  • Provide care in client's homes and clinic settings, with a focus on delivering effective wound care management and diabetes management.
  • Travel around South Dublin necessary for this post, with staff eligible to claim travel allowance for work-related travel.

The service is provided Monday to Friday, 9-5, with hours negotiable and part-time options available. Onsite training and orientation are provided by HSE staff.

Responsibilities:
  • Deliver nurse-led wound care management in clinic and domiciliary settings, with a focus on achieving positive patient outcomes.
  • Manage diabetes care for clients, working closely with multidisciplinary teams to ensure effective care delivery.
  • Conduct incontinence assessments and provide appropriate management plans, ensuring optimal patient comfort and well-being.
  • Perform nursing assessments post hospital discharge/GP referral, identifying areas for improvement and implementing strategies to enhance patient care.
  • Provide general nursing care for chronically ill or disabled patients, prioritizing their needs and delivering high-quality care.
  • Offer oncology support and end-of-life care, providing emotional support and guidance to patients and their families.
  • Conduct nursing assessments for home supports, ensuring that patients receive the care they need to live safely and independently at home.

Please note that access to own transport is required for this post, and work authorisation is mandatory. NMBI registration as an RGN and community nursing experience are preferable, along with acute experience working as a general nurse and excellent wound care experience.
