Dublin North

Up to €38,000 (based on experience & qualifications)
Hours:
Monday–Friday, 9:00am to 5:00pm/8:00am to 4:00pm with 1-2 weekend days per month, 2 evenings per month.

Client Care Coordinator
who shares our values to help us support clients across
Dublin North City & County.
This role is perfect for someone who enjoys a mix of
teamwork, problem-solving, people interaction
, and a dash of hands-on care when needed
.
What You'll Do
Coordinate and manage
care services for a group of amazing clients.
Liaise with clients and families
, offering support and guidance with warmth and empathy.
Create and maintain
person-centred care plans
, updating them as needs change.
Mentor and support
our Healthcare Assistants to deliver top-notch care.
Keep care records up to date using our
OneTouch Health
system.
Visit clients for reviews, assessments and provide hands-on care provision.
Ensure everything we do meets our high standards and HSE/HIQA regulations.
What You'll Need
Experience in
care coordination
in a home care setting.
QQI Level 5 Major Award
in Healthcare Support (which must must
Care Skills
and
Care of the Older Person
) & a relevant qualification at
QQI Level 6
or higher.
Confident using Microsoft Office and care software (OneTouch Health experience is a bonus).
Great communication and people skills – you'll be talking to clients, families and care staff daily.
Excellent organisation and time management – every day is different
Full driving licence & access to a car
(travel allowance provided).
Eligibility to work in Ireland without requiring a work permit.
