As a Rehabilitation and Aftercare Service Coordinator, you will work closely with our team to deliver high-quality services that support individuals in their recovery journey.
Main Responsibilities
- Service Coordination
- Coordinate the development and implementation of our rehabilitation and aftercare service, ensuring alignment with the National Drug Strategy (Reducing Harm, Supporting Recovery 2017 – 2025).
- Cultivate partnerships with local organizations to enhance service delivery.
- Client Engagement
- Engage with clients, understanding their needs and preferences to develop tailored support plans.
- Collaborate with the Family Support Worker to facilitate family-centered care.
- Reporting and Quality Assurance
- Analyze client data to identify trends and areas for improvement.
- Contribute to quality assurance initiatives, ensuring our services meet the highest standards.
Qualifications and Experience
- A relevant third-level qualification (minimum Level 7 or equivalent) in social care, addiction, or a related field.
- A minimum of 2 years' experience in addiction, community development, health, and social care.
