Key Responsibilities:
  1. Build trust-based relationships with service users, facilitating their self-identification of goals, strengths, and needs.
  2. Support service users in establishing and enhancing their natural support networks.
  3. Collaborate with service users to identify community-based resources and develop practical solutions to barriers.
  4. Provide accessible information on available community supports, ensuring activities and services are tailored to individual needs.
  5. Advocate for and support self-advocacy among service users in various settings, including family, healthcare providers, and community groups.
  6. Develop and maintain positive risk assessments.
  7. Research and create Community Profiles, sourcing local activities and initiatives.
  8. Foster partnerships with local organizations to enhance community capacity and inclusivity.
  9. Facilitate ongoing review of service user progress and adjust strategies as needed.
  10. Engage in activities to motivate service users and promote participation.
Requirements:
  • A third-level graduate or equivalent qualification in health and social care, preferably at level 7 or above.
  • Experience working with adults with disabilities and designing personalized programs.
  • Proven track record of building, leading, and motivating teams.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with stakeholders.
  • Knowledge of person-centered planning, facilitation, and implementation.
  • Awareness of budget management and cost control principles.

Additional Requirements: A full driving license and flexibility regarding working hours and travel are essential. Being action-oriented, adaptable, and willing to take on new challenges is vital for this role.
