Job Title: Care & Case Manager

This is a challenging and rewarding role that requires strong communication skills, empathy, and the ability to work independently.

The successful candidate will be responsible for providing support to tenants, assessing their needs, and developing personalized care plans. They will also be required to maintain accurate records, manage budgets, and coordinate with external agencies.

Key Responsibilities:
  • To provide individualized support to tenants, taking into account their unique needs and circumstances.
  • To assess the needs of tenants and develop personalized care plans that meet their goals and objectives.
  • To maintain accurate and up-to-date records of tenant information, including assessments, care plans, and progress notes.
  • To manage budgets and financial resources effectively, ensuring that they are allocated in a way that supports the delivery of high-quality services.
  • To coordinate with external agencies, including healthcare providers, social services, and other organizations that may be relevant to the provision of support services.
Requirements:
  • Degree or diploma in Social Care or related field.
  • Minimum 2 years' experience in residential social care services, preferably in the homeless/supported housing sector.
  • Leadership experience in social care services, including supervision of social care staff would be a distinct advantage.
  • Extensive experience in working with behaviours that may challenge and supporting care-staff in similar environments.
