About the Role: Mowlam Healthcare Group is currently seeking a highly skilled and experienced Patient Flow Manager to join our team. As a key member of our Patient Flow team, you will be responsible for ensuring the safe and effective management of resident and patient referrals to Leopardstown Care Centre.

Key Responsibilities:

  • Receive patient referrals from acute hospitals and review them in conjunction with referral teams and discharge coordinators
  • Completion of pre-admission assessments to ensure suitability for placement
  • Liaise with doctors regarding admissions, prescriptions, and admission paperwork
  • Facilitate patient admissions and discharges
  • Maintain accurate and informative records in accordance with legislation and healthcare standards

Requirements: To be successful in this role, you will require 3 years of post-registration experience in a relevant field, including experience in managing patient caseloads. You must also hold a recognised managerial qualification and be registered on the NMBI live register.
