Key Responsibilities
- To promote and support independent living and community integration, we will develop and deliver personalized programs tailored to the needs of our service users.
- To facilitate educational, developmental, recreational, and occupational programs and activities, ensuring they align with the service user's goals and capabilities.
- Identify goals and supports required by individual service users, maintaining accurate records and collaborating with the Programmes Supervisor/Community Services Manager as necessary.
- Support individuals and groups in accessing their community, facilitating activities and programs that meet identified needs.
Required Skills and Qualifications
- Minimum QQI Level 5 in Social Care or equivalent qualification.
- Good knowledge of New Directions and community-based programs.
- Understanding of intellectual disabilities and mental health issues.
- At least one year of experience working in a relevant setting, with experience in facilitating and implementing person-centered plans.
