The role involves supporting service users in making informed choices, developing programmes within a person-centred model of service delivery. Programmes may include independent living, personal development, and occupational and leisure activities, aligned with a quality-of-life model.
Your key responsibilities will include:
- Being a key worker to service users, identifying goals and supports required by individual service users;
- Developing and delivering person-centred programmes;
- Supporting individuals and groups in accessing their community;
- Liaising with relevant team members;
- Communicating with external agencies and families;
- Maintaining records;
- Observing rules and regulations;
- Promoting independence and community integration;
- Ensuring the safe transportation of clients.
We are looking for someone who has experience supporting adults with intellectual disabilities and mental health issues, and who can approach interactions with service-users in a professional manner and with dignity and respect.
