Job Opportunity: Client Care Coordinator

About the Role

This role involves ensuring high-quality care delivery by coordinating services, managing schedules, and communicating effectively with clients, care teams, and healthcare professionals.

  • Scheduling: Coordinate client care schedules to ensure appropriate coverage and make adjustments as necessary.
  • Communication: Liaise between clients, their families, care team members, and healthcare professionals to facilitate smooth communication.
  • Client Support: Address client inquiries, concerns, and requests, providing support and guidance to meet their needs.
  • Record Keeping: Maintain accurate and up-to-date client records, documentation, and care plans to ensure seamless service delivery.
  • Quality Assurance: Monitor client care delivery to ensure adherence to high standards of care.
  • Administrative Tasks: Assist with data entry, filing, report generation, and other administrative duties to optimize office efficiency.

Requirements

To succeed in this role, you should possess excellent communication and interpersonal skills, both verbal and written. Strong organisational skills are also essential for managing multiple tasks and maintaining accurate records. Additionally, you should be able to problem-solve effectively to assess situations and implement solutions. Proficiency in Microsoft Office Suite (Word, Excel, Outlook) and relevant software is a plus.

  • Prior healthcare experience or working with individuals in need of care is desirable.
