About the Opportunity

The model of service is mainly home-based with interventions provided including falls prevention, cognitive rehabilitation, community skills training, and self-care reablement. The occupational therapist will also provide clinic-based services such as lifestyle redesign groups, assistive technology training, complex seating assessments, and upper limb rehabilitation. The primary focus is on helping the client to achieve their maximum potential in the areas of functional participation, health promotion, and occupational wellness.

We are seeking an experienced occupational therapist to join our multidisciplinary team delivering a coordinated approach to client care. You will work closely with other healthcare professionals to develop and implement effective treatment plans that meet the unique needs of each client. Your expertise will be valued as you contribute to the growth and development of our occupational therapy department within Community Healthcare Services in West Clare.

Key Responsibilities:
  • Assess, plan, implement, and review treatment programs
  • Manage caseload and collaborate with service users, families, and staff in treatment planning
  • Plan service user discharge or transition appropriately
  • Document assessments, treatment plans, and progress notes according to standards
  • Communicate assessment results and treatment recommendations to the team
