Social Care Specialist

About the Role:
This is an exceptional opportunity to join our dynamic team as a Social Care Specialist, working with young people aged 15-18 in our Under 18s Residential Services. As a key worker, you will provide critical support and guidance to these young individuals, empowering them to achieve their goals and overcome challenges.
Your primary focus will be on delivering person-centred care, working collaboratively with other teams, external agencies, and stakeholders to ensure the best possible outcomes for our clients. You will also be responsible for maintaining accurate records and participating in team meetings.
Main Responsibilities:
- Develop and implement individualised programmes to support young people's experiences of homelessness, promoting positive change and preparing them for independent living.
- Collaborate with external agencies to address young people's housing, social work, education, training, and legal needs.
- Provide advocacy services to young people, ensuring their rights are respected and protected.
- Support young people in developing and maintaining individual placement plans, aligned with statutory care plans.
Requirements:
To excel in this role, you will need:
Significant experience in a similar role or equivalent environment.
A strong academic background, preferably in Social Care or a related field.
A full clean driving license.
An excellent understanding of child protection policies and procedures.
Excellent communication and interpersonal skills.
