As a Care & Case Manager, you will play a key role in supporting our service users to maintain their tenancies and achieve their goals. This is an exciting opportunity to work with a dedicated team and make a real difference in the lives of our service users.

The successful candidate will be responsible for assessing the needs of service users, developing and implementing care plans, and providing support to help them achieve their goals. You will also be required to communicate effectively with service users, their families, and other stakeholders to ensure that their needs are met.

In addition to your core responsibilities, you will also be required to participate in regular team meetings, contribute to the development of new services, and undertake ongoing training and professional development to maintain high standards of practice.

  • Qualifications: National Diploma in Applied Social Care Studies, Diploma in Social Care, or equivalent qualification">
  • Experience: Not less than 2 years experience in residential social care services, preferably in the homeless/supported housing sector">
  • Leadership Experience: Supervision of social care staff would be a distinct advantage">
  • Skills: Excellent communication skills, report writing, full clean driving license">

Eligibility Criteria: Candidates must have a suitable standard of professional attainments and requisite knowledge and ability, including a high standard of suitability and ability for the proper discharge of the duties of the office.

Post Specific Requirement: A thorough knowledge of principles of Social Care/Community Work, health and safety legislation, data protection, report writing, and funding structures for homeless services.
