Job Title: Care Manager

The jobholder will assist in the day-to-day running of a service while adhering to set guidelines. This role requires effective communication and problem-solving skills.

  • Processing referrals, conducting assessments, care planning, and implementing support plans;
  • Engaging with clients, supporting them with housing sustainment, addressing maintenance issues, and ensuring rent payments are made;
  • Collaborating with local authorities and other agencies;
  • Providing out-of-hours support when designated.

Accountabilities and Indicators:
  • To demonstrate high standards of social care practice, adhering to tenancy law;
  • Ensuring the service responds effectively to client needs in areas such as referral, induction, key working, and property law and maintenance;
  • Developing life and home-making skills and ensuring access to community supports and welfare allowances;
  • Maintaining high-quality documentation standards and observing professional standards in respect of confidentiality and data security;
  • Participating in research projects and contributing to service improvement plans.
