Job Title: Community Health Specialist

About the Role:
  • The successful candidate will be responsible for working as a Community Health Specialist in the HSE department.
  • Location: Lifford, St Johnston Newtowncunningham area in East Donegal.
  • This is a full-time requirement, Monday to Friday service with potential weekends included.
  • Hours of work will be from 9am to 5pm.
  • This role is an agency line of work with Department of health rates applying.
  • Possession of a car for travel is essential for this position.
Key Responsibilities:
  • Deliver community-based health services and initiatives.
  • Work collaboratively with healthcare teams to ensure seamless delivery of care.
  • Develop and maintain relationships with key stakeholders, including local communities, healthcare providers and other relevant parties.
  • Conduct regular assessments and evaluations to identify areas for improvement and develop strategies to address these.
  • Participate in quality assurance and quality improvement activities to ensure high-quality service delivery.

Requirements:
  • A degree in a relevant field such as nursing, social work or public health.
  • Proven experience in a similar role or industry.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong analytical and problem-solving skills.
  • Familiarity with relevant policies, procedures and regulations.
