Job Opportunity

The Care and Case Manager will be responsible for supporting the day-to-day operation of our service.

  • Processing referrals and conducting assessments to identify individual needs.
  • Developing and implementing care plans that cater to diverse requirements.
  • Maintaining strong relationships with tenants through personalized housing sustainment support.
  • Addressing any issues arising from tenancy maintenance or payment concerns.

This role will also play a significant part in Case Management, Tenancy Sustainment, and Resettlement efforts. Collaboration with local authorities is crucial for this position's success.

As an integral part of our team, the post-holder will represent the Service Manager and provide out-of-hours on-call support when necessary.

Key Responsibilities
  • To conduct comprehensive assessments of clients' holistic needs and refer them to relevant support services as needed.
  • To demonstrate exceptional standards of social care practice and adherence to tenancy laws.
  • To foster a positive and compliant rent culture among tenants.
  • To ensure the service responds effectively to the needs of its users.
  • To continually strive for delivering high-quality care to tenants.
Essential Requirements
  • National Diploma in Applied Social Care Studies or equivalent qualification.
  • A minimum of 2 years' experience in residential social care services, preferably within the homeless/supported housing sector.
  • Leadership experience in social care services, including supervision of social care staff.
