Geriatric Care Specialist

The Geriatric Care Specialist is responsible for delivering high-quality nursing care to residents in a rehabilitation setting. The successful candidate will assess, plan, implement, and evaluate individualized care plans to meet the unique needs of each resident.
Responsibilities
- Assess the basic nursing care needs of geriatric residents and develop effective care plans to address these needs.
- Provide direct patient care, including administering medications, performing treatments, and providing emotional support to residents and their families.
- Collaborate with interdisciplinary teams to ensure seamless coordination of care and optimal outcomes for residents.
- Monitor and report changes in resident condition to ensure timely interventions and prevent adverse events.
- Participate in quality improvement initiatives to enhance the overall quality of care and services provided to residents.
Qualifications
- Current licensure as a Registered Nurse in the state where the position is located.
- Minimum one year experience in a geriatric or rehabilitation setting.
- IV Certification required or willingness to obtain within six months of employment.
- Current CPR/AED Certification required or willingness to obtain within six months of employment.
- Excellent communication and interpersonal skills.
- Strong organizational and time management skills.
- Critical thinking and problem-solving abilities.
