About the Role:

As a Client Care Manager, you will be responsible for assessing client needs, developing personalised care plans, and coordinating care services to enhance their well-being. You will work closely with clients, families, caregivers, and healthcare professionals to provide exceptional support and ensure care plans are delivered to the highest standards.

Key Responsibilities:

  • Conduct comprehensive assessments of clients' needs and create tailored care plans in collaboration with families and healthcare teams.
  • Oversee and coordinate the implementation of care services, ensuring compliance with regulations.
  • Regularly review and adjust care plans to meet evolving client needs.
  • Provide guidance and training to caregivers, ensuring they have the necessary resources to deliver quality care.
  • Act as the primary point of contact for clients and their families, addressing concerns in a compassionate and timely manner.
  • Work closely with an interdisciplinary team to deliver holistic, person-centred care.
  • Maintain accurate records of client assessments, interactions, and care plans.

What We're Looking For:

  • QQI Level 6 in Health Services Supervisory Management
  • Experience in a healthcare or homecare setting
  • Strong communication and interpersonal skills to build trust with clients and families
  • Excellent organisational and problem-solving abilities
  • Ability to work independently and collaboratively in a dynamic environment
  • Proficiency in Microsoft Office and electronic health record systems
  • Full driver's license and access to a reliable vehicle (preferred)
