OverviewThe CNS/CMS post holder will deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse/Midwife Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM Patient caseload includes Haematology/Oncology/Radiotherapy patients over 16 years of age.
Responsibilities- Direct care: Provide a specialist nursing/midwifery service for service users with a cancer diagnosis, including comprehensive assessments (physical, psychological, social and spiritual) and development of care plans in collaboration with the MDT.
- Direct care: Monitor and evaluate patient responses to treatment; amend care plans in consultation with the MDT, patient, family and/or carer; coordinate investigations, treatments and follow-up; participate in unscheduled reviews of patients presenting with disease/treatment-related side effects.
- Direct care: Accept referrals from MDT colleagues and provide a telephone triage service using the UKONS tool; support self-management and symptom management strategies.
- Direct care: Manage nurse/midwife led Oncology clinics in collaboration with the MDT; promote health education and prevention.
- Indirect care: Identify, agree and document referral pathways; participate in case reviews; use a case management approach with Primary and Secondary Care as appropriate; develop and update PPPGs and service standards.
- Advocacy: Act as a service user advocate, communicate and represent patient/family values to MDT; promote patient involvement in decision making; protect privacy and confidentiality.
- Education & Training: Maintain clinical competence; provide information and education to service users, families and carers; contribute to design and delivery of education programmes; mentor and preceptorship; participate in MDT training; develop links with education providers and NCCP in Acute Haematology Oncology.
- Audit & Research: Establish quality metrics; maintain KPI records; conduct and participate in nursing/midwifery audits and research; disseminate evidence to improve practice; contribute to service planning and budget processes; monitor and use current research to inform practice.
- Consultancy/Leadership: Act as a resource and role model; contribute to clinical standards and guidelines; build collaborative relationships with CNSs and MDT colleagues; engage in integrated care planning; liaise with voluntary organizations and other health service providers; support professional development and teaching.
- Health & Safety: Comply with HIQA standards and HSE protocols; participate in sustainable initiatives; ensure safety procedures are followed per Health, Safety and Welfare at Work Act 2005 and local safety statements.
- Quality, Risk and Safety: Participate in quality, risk and safety initiatives; identify and manage risk; support audits and incident reporting; develop PPPGs and safe practice; comply with data protection requirements.
- Hygiene: Ensure compliance with local hygiene standards, guidelines and practices.
- Management/Administration: Provide efficient, high-quality nursing service; manage time and caseload; maintain records in line with HSE requirements; participate in service planning; implement governance and risk management; lead development of policies and guidelines; promote health and self-management; ensure data protection compliance; support sustainability initiatives.
