Purpose of the RoleThe post holder will be responsible for the provision of the highest standard of nursing care to patients, manage all nursing and allied staff in the ward/unit, and be responsible for maintaining and developing professional standards.
Key Duties and Responsibilities- Assist and signpost staff with safe discharges of patients from the acute hospital to their home.
- Promote the continued care of patients in the community after their acute phase.
- Link, liaise and forge strong relationships and alliances with our community partners in particular, our Public Health Nurse (PHN) and Chronic Disease Hub (CDH) colleagues.
- Provide efficient and effective management in such a way as to provide a safe environment for patients and staff, while making the best use of resources.
- Promote collaboration within the multidisciplinary team. Ability to work independently and collaboratively with diverse populations.
- Engage with key stakeholders to ensure hygiene standards are maintained for patients, staff and visitors.
- Proactively manages quality and safety standards.
- Adhere to Code of Professional Practice and Behaviours for registered nurses.
- Show leadership potential to make an impact, influence, energise and inspire others across the directorate and pan organisation.
- Embrace change and act to implement evidence-based best practice where appropriate.
- Adhere to hospital and/or professional standards, policies and procedures relevant to their area of practice.
- Communicate effectively in English, written and spoken, as appropriate to job requirements.
- Participate and cooperate with legislative and regulatory requirements in relation to Health and Safety as outlined in the Safety, Health and Welfare Act 2005 and the Hospital Safety Statement.
- Ensure own mandatory learning and training is kept up to date.
- Flexible to work in the Hospital/community environment as required with knowledge of public health principles and community resources.
- Safe discharges of patients from SJH in collaboration with Bed Management and the multidisciplinary Teams.
- Improve egress to optimise patient flow.
- Problem solving.
- Education of staff members on community services available from PHN and CDH and the wider community team as needed.
- Create content to keep staff up to date on Community services.
- Clinical audit and metric setting, monitoring, and evaluating of key performance indicators.
- Actively participate in research and quality improvement plans.
- Engage in Magnet4Europe and Pathway to Excellence.

Required- Registered General Nurse with The Nursing and Midwifery Board of Ireland (NMBI), or eligible to register.
- 5 years post-registration experience in an acute hospital setting or relevant clinical area within the last 7 years.
- Demonstrate management experience within the last 2 years.
- Experience in teaching and assessing the nursing and HCA team within the last 2 years.
- Management course.
- Community management experience.
- Working knowledge of healthcare patient record systems.
