We are looking for an experienced Clinical Nurse Manager, Patient Flow to join a Nursing Home in Dublin. Requirements: Registered General Nurse (RGN) with an active NMBI registration. At least 2 years of experience in a nursing home setting. Patient flow experience. Experience as a Clinical Nurse Manager (desirable). Demonstrated leadership ability and experience in managing nursing staff. Strong communication, interpersonal and team-building skills. Excellent organizational and decision-making capabilities. Comfortable working in a dynamic environment and managing multiple priorities. Right to work in Ireland (Stamp 4 or EU Citizenship). Key Responsibilities: Coordinate admissions, discharges, and internal transfers to ensure efficient patient flow and optimal bed management. Collaborate with multidisciplinary teams, families, and external partners to support timely and appropriate transitions of care. Oversee clinical assessments and care planning to ensure safe, person-centred placement decisions. Monitor compliance with HIQA standards and maintain accurate documentation throughout the patient journey. Provide regular reporting on occupancy, referrals, and flow metrics to support service planning and decision-making.
