The role of a Social Care Worker is to provide support and care for individuals with intellectual disabilities, autism, complex medical needs, mental health, behavioral challenges, and physical disabilities. The successful candidate will work in a team environment, providing a safe and secure environment where individuals can participate in community and leisure activities, make choices, and enjoy a lifestyle that fulfils their potential and expectations.

In this role, the Social Care Worker will be responsible for reporting to the Social Care Leader/Team Leader and fulfilling a key worker role for individuals. This includes providing individualized support, implementing care plans, and promoting independence and choice. The successful candidate will also be required to work collaboratively with other professionals, including healthcare workers, therapists, and educators.

To be successful in this role, candidates must have a professional registration on the Social Care Workers Registrar maintained by the Social Care Workers Registration Board at CORU. They should also have a minimum of one year's experience of working with people with intellectual disabilities, as well as excellent organizational and planning skills, effective communication skills, and the ability to exercise good judgement and discretion.

Responsibilities:

  • Provide individualized support and care to individuals with intellectual disabilities
  • Implement care plans and promote independence and choice
  • Work collaboratively with other professionals to achieve positive outcomes
  • Monitor and record progress, identifying areas for improvement
  • Participate in staff training and development programs

Requirements:

  • Professional registration on the Social Care Workers Registrar
