Job Title: Care & Case Manager

Care and case managers play a vital role in ensuring the well-being of vulnerable individuals. The position involves working closely with service users to assess their holistic needs, provide support, and connect them with relevant resources.

As a care and case manager, you will be responsible for:

  • Supporting tenants with their tenancy maintenance and rent adherence
  • Assessing holistic needs and referring service users to appropriate supports
  • Informing tenants of their rights and responsibilities
  • Providing out-of-hours on-call support where designated

Qualifications and Experience: A minimum of 2 years' experience in residential social care services is required. Leadership experience and knowledge of social care/community work principles are advantageous.

Skills and Competencies: Excellent communication skills, leadership ability, and a full clean driving license are essential for this role.
