
Care Coordination Specialist

We are seeking a highly skilled and experienced nursing professional to join our team in providing exceptional care to our residents.
- Main Responsibilities:
- Support the Director of Care, Clinical Nurse Manager/Snr. Staff Nurse in their role across the span of his/her duties.
- Maintain professional and ethical standards and set a high standard of resident care.
- Adhere to NMBI guidelines on the administration of medical preparations and the Drugs Act.
- Work as part of a team having regard to the individual team members and an appreciation of their contribution to the team.
- Maintain excellent working relationships with staff at all levels within the organisation.
- Maintain strict confidentiality of all information acquired during the course of your work.
- Deliver Induction training to new nursing and care staff.
- Ensure that each resident has individualised assessments based on their specific needs, that each resident has a person centred care plan implemented, evaluated and modified as their care needs change.
- Ensure all nursing care is individualised and holistic to meet resident's needs.
- Develop and promote good relationships with residents and their families communicating effectively with them and ensuring that they are informed of any changes as appropriate.
- Supervise and allocate duties to care assistants.
