Homelessness Support Specialist

The primary responsibility of this role is to support individuals who are experiencing homelessness by providing comprehensive case management services. This includes assessing the needs and strengths of each individual, developing personalized plans for service delivery, and facilitating access to mainstream and specialist services.
Key Responsibilities:- Conduct assessments of individuals' needs and strengths to identify potential barriers to housing stability
- Develop and implement individualized plans for service delivery, including housing stabilization and tenancy support
- Facilitate access to mainstream and specialist services, such as healthcare and addiction treatment
- Provide key worker support to individuals in achieving their goals and aspirations
- Work collaboratively with other agencies and stakeholders to ensure seamless service delivery
To be successful in this role, you will require a strong understanding of social care principles and practices, as well as excellent communication and interpersonal skills. You will also need to have experience working with vulnerable populations, particularly those experiencing homelessness.
- Qualifications: Diploma in Applied Social Studies or equivalent
- Experience: Minimum 2 years' experience working in residential social care services, preferably in the homeless sector
- Skills: Strong understanding of social care principles and practices, excellent communication and interpersonal skills, ability to work with vulnerable populations
