Job Opportunity: Client Care Coordinator Role in Dublin North WestWe are seeking a skilled and compassionate Client Care Coordinator to join our team. As a key member of our team, you will play a vital role in supporting clients across Dublin North City & County.The successful candidate will be responsible for coordinating and managing care services for a group of amazing clients. This will involve liaising with clients and families, offering support and guidance with warmth and empathy.You will create and maintain person-centred care plans, updating them as needs change. Additionally, you will mentor and support our Healthcare Assistants to deliver top-notch care.Key aspects of this role include:
  • Experience in care coordination in a home care setting.
  • QQI Level 5 Major Award in Healthcare Support (which includes Care Skills and Care of the Older Person) & a relevant qualification at QQI Level 6 or higher.
  • Confident using Microsoft Office and care software (OneTouch Health experience is a bonus).
  • Great communication and people skills – you'll be talking to clients, families and care staff daily.
  • Excellent organisation and time management – every day is different.
  • Full driving licence & access to a car (travel allowance provided).
  • Eligibility to work in Ireland without requiring a work permit.
