Galway Simon Community is seeking a compassionate and motivated nurse to join their Homeless Services team.

The successful candidate will work within a multidisciplinary team, delivering a coordinated approach to client care and coordinating services for particular cases as required.

They will play a key role within the Housing First Team, working to ensure that very vulnerable clients can access the Health Supports that they need.

This role will include delivery of first aid training to PHECC standards of compliance. Training for this will be provided if required.

Key Responsibilities
  • Practice Nursing according to the Code of Professional Conduct as laid down by An Bord Altranais agus Cnáimhseachais na hÉireann (Nursing Midwifery Board Ireland) and Professional Clinical Guidelines.
  • Act as a First Aid instructor, training and certifying employees in first aid best practice.
  • Comply with national, regional and local Health Service Executive guidelines, policies, protocols and legislation.
  • Work within the scope of practice and take measures to develop and maintain the competence necessary for professional practice.
  • Maintain a high standard of professional behaviour and be accountable for his or her practice.
  • Be aware of ethical policies and procedures which pertain to the area of practice.
  • Respect and maintain the privacy, dignity and confidentiality of the client.
Clinical Practice
  • Fulfil statutory obligations within the legislation, HSE policies as appropriate to the role.
  • Promote the health, welfare and social wellbeing of all clients.
  • Manage a caseload as appropriate and evaluate and develop services within this caseload through a population health approach.
  • Facilitate Nurse led clinics within homeless services, Advocate on behalf of clients to ensure they obtain a GP and Medical Card.
  • Discharge planning once a client is hospitalised, working with the multi-disciplinary team and follow-up post discharge.
  • Nursing assessments for clients for Fair Deal funding and Home-help services.
  • Provide education and information to the clients, as required and be an advocate for the individual client.
Education, Training & Development
  • Take responsibility for own learning and development needs and actively contribute to the learning and development of those they work with.
  • Develop and use reflective practice techniques to inform and guide practice.
  • Participate in regular performance/clinical reviews with the line manager.

Requirements
  • Candidates must be Registered by An Bord Altranais agus Cnáimhseachais na hÉireann (Nursing Midwifery Board Ireland) or be so entitled to be registered as a RGN and/or PHN.
  • Demonstrate a high-level of knowledge and experience to carry out the duties and responsibilities of the role.
Desirable
  • Experience working with people who are marginalised or socially excluded will be a distinct advantage.
  • A valid First Aid Response Instructor Cert will be a distinct advantage.
