Patient Care Specialist

Our organization is seeking a compassionate and dedicated individual to provide high-quality patient care in a community-based setting. This permanent position offers flexible working hours and the opportunity to make a meaningful difference in the lives of our clients.
Key Responsibilities:- Deliver personalized care and assistance to clients based on their Care Plan, including personal care, meal preparation, and supervising medication.
- Maintain a client-centric approach that prioritizes and safeguards the well-being and interests of clients at all times.
- Regularly report significant changes in a client's physical, behavioral, or social condition to the Line Manager.
- Perform a range of Personal and Practical Care Duties with professionalism and empathy.
- FETAC/QQI Level 5 Award or equivalent qualification in Care Skills and Care of the Older Person modules.
- A valid driver's license and access to a personal vehicle for work-related purposes.
- A minimum of one year of relevant experience within the healthcare industry.
