Critical Case Manager Job Description

As a Critical Case Manager, you will be part of a team providing professional services to families experiencing homelessness.

The purpose of this role is to assess and support families through case management and triage supports. You will provide families with the skills to maintain emergency accommodation and move out of homelessness into long-term accommodation.

Key Responsibilities:
  • Undertake assessments and develop/implement support plans with families experiencing homelessness.
  • Implement support plans to assist families to maintain emergency accommodation.
  • Liaise and advocate with relevant authorities to support families.
  • Manage an allocated caseload and operate a triage support service.
  • Provide advice, information, and advocacy for families experiencing homelessness.
  • Identify customers' needs and support the development of services to meet these needs.
  • Work within the framework of the overall objectives of your employer.
  • Work with and under the supervision of line management.
  • Work as part of a team in delivering services.
  • Respond to phone queries, emails, and families presenting to the service seeking support.
  • Work within a client-centred approach, encouraging autonomy and independence.
  • Maintain accurate records and statistics.
Requirements:
  • Relevant third-level degree in social care or a related area.
  • At least two years' experience in the social care field working with disadvantaged individuals and families.
  • Experience of supporting marginalized households with high-risk behavior.
  • Proven advocacy skills and ability to work on own initiative.
  • Ability to work alone and as part of a team.
