Job Title: Support Facilitator

We are seeking a qualified and experienced Care Support Facilitator to join our team. The ideal candidate will have training in Manual Handling, Patient Handling, or First Aid, as well as at least 1-2 years' experience in a role providing direct support or care.

Main Responsibilities:
  1. Develop Individualised Support Plans: Create and implement tailored support plans that cater to the unique needs and goals of adults receiving care.
  2. Coordinate Services: Act as the primary liaison between healthcare professionals, housing services, social care agencies, and community organizations to ensure seamless coordination of support.
  3. Risk Assessment and Management: Conduct thorough risk assessments to identify potential hazards and develop strategies to mitigate them, ensuring a safe environment for adults receiving care.
  4. Outcome Tracking and Performance Monitoring: Regularly review and analyze data to track outcomes and performance measures, making adjustments as necessary to optimize service delivery.
  5. Advocacy and Partnerships: Provide advocacy services to adults receiving care, working closely with partners to address challenges and promote effective solutions.
  6. Team Collaboration and Communication: Foster strong relationships with healthcare professionals, colleagues, and other stakeholders to deliver high-quality, coordinated care.
Required Skills and Qualifications:
  • Genuine Passion for Supporting Adults: Demonstrate a genuine enthusiasm for working with adults and supporting their independent living.
  • Professionalism and Discretion: Handle sensitive information with professionalism and discretion, maintaining confidentiality when required.
  • Strong Interpersonal Skills: Possess excellent communication and interpersonal skills, with confidence working alongside multiple stakeholders.
  • Problem-Solving and Adaptability: Develop and implement effective solutions to meet changing needs, demonstrating adaptability and a proactive approach.
  • Administrative and Organisational Skills: Maintain accurate records and reports, utilizing organisational information systems effectively.
  • IT Literacy and Case Management Software: Familiarity with IT systems, including case management software, is desirable.
  • Listening Skills and Commitment to Quality: Provide attentive listening services and demonstrate a commitment to delivering high-quality care.
