As a Community Health Assistant, you will be responsible for delivering high-quality care to clients in their own homes. This is a rewarding role that requires strong interpersonal skills and the ability to work independently.

Key Responsibilities:

  • Deliver person-centred care and support to clients with varying needs and requirements
  • Conduct risk assessments and develop appropriate care plans in consultation with colleagues
  • Participate in multidisciplinary team meetings to review client progress and plan future interventions
  • Develop and maintain effective working relationships with clients, families, and other healthcare professionals
  • Stay up-to-date with relevant legislation, policies, and procedures affecting your role

Requirements:

  • A valid HCA qualification or equivalent experience
  • Excellent communication and interpersonal skills
  • The ability to work independently and as part of a team
  • A flexible approach to working hours, including evenings and weekends
  • Reliable and punctual attendance
