Job Title: Social Care Worker

  • The successful candidate will be responsible for developing and implementing personalized plans to meet the ongoing needs of service users.
  • They will also contribute to plan development and ensure that changes are implemented as necessary.
  • Strong interpersonal and communication skills are essential for effective collaboration with colleagues and service users.
  • Proficiency in Microsoft Office and Teams is also required.
