Job Title: Health Services Coordinator

As a Health Services Coordinator, you will play a vital role in providing community-based integrated physical and behavioral health services to individuals with complex and chronic mental illness, substance abuse, and medical comorbidity.

You will work as part of an interdisciplinary team to identify chronic behaviors, discuss their impact, and develop improvement strategies and specific goal-directed interventions.

The ideal candidate will have excellent communication skills, be able to work effectively within a team environment, and possess a strong understanding of the importance of patient-centered care.

Responsibilities:

  • Work within a team caseload of individuals with co-occurring physical and mental health needs.
  • Meet regularly with individuals served in office and community settings.
  • Participate in the selection of strategies to implement evidence-based wellness and prevention initiatives.
  • Develop care plans including specific goals for all enrollees.
  • Communicate with medical providers, subspecialty providers, and other healthcare professionals regarding records and treatment plans.
  • Provide education on health conditions, treatment recommendations, medications, and strategies to implement care plan goals.
  • Monitor assessments and screenings to ensure findings are integrated in the care plan.
  • Collect accurate vital signs as part of assessment/ongoing monitoring.
  • Complete Point of Care testing when indicated for ongoing monitoring of psychiatric, substance use, and/or medical conditions.
  • Assess and recommend laboratory testing based on medications, psychiatric, and medical conditions.
  • Facilitate the use of the Electronic Health Record (EHR) and other Health Information Technology (HIT) to link services and facilitate communication among team members.
  • Attend regular care team meetings to plan care and discuss cases.

Qualifications:

  • Current unrestricted RN license.
  • Valid and unrestricted Driver's License.

Preferred qualifications include a Bachelor of Science in Nursing (BSN) degree and one year of experience working with adults in a mental health setting.
