
Community Support Specialist

At our Bridgewater Resource Centre in Clonmel, Tipperary, we are seeking an experienced Support Worker to join our team. This is a unique opportunity for someone who wants to make a positive impact on the lives of individuals with disabilities and their families.
We are looking for an individual who can develop and deliver person-centred programmes that meet the needs of service users as identified through Person Centred Planning.
The successful candidate will be responsible for:
- Developing and delivering programmes to support individuals and groups in accessing their community
- Maintaining records and being a key worker to service users to facilitate the Person Centred Planning process
- Identifying goals and supports required by individual service users and maintaining records as required
Requirements:
- Minimum QQI Level 5 in Social Care, or equivalent qualification
- Good knowledge of New Directions and community-based programmes
- Knowledge and understanding of intellectual disabilities and mental health issues
- 1 year minimum experience of working in a relevant setting with experience of facilitating and implementing person-centred plans
