Care Coordinator for Adult Outpatient Services

As an Occupational Therapist, you will provide high-quality care to adult outpatient patients with orthopedic or neurological conditions in our clinic locations. Your role will involve evaluating individuals referred for occupational therapy, establishing prognosis and treatment plans, implementing OT treatments based on evaluations and plans of care, and re-assessing ongoing treatment effects.
Key Responsibilities:
- Evaluate individuals referred for occupational therapy
- Establish prognosis and treatment plans
- Implement OT treatments based on evaluations and plans of care
- Re-assess ongoing treatment effects
- Collaborate with healthcare professionals involved in patient care
- Supervise occupational therapy assistants and aides
