We are seeking a skilled Addiction Practitioner to join our team in North Dublin City and County.
Key Responsibilities:- Provide one-to-one key working and case management using evidence-based interventions to support individuals struggling with problematic alcohol use.
- Develop and implement client-centred care plans for clients with dependent alcohol use, including managing their caseload and providing referrals to relevant services.
- Engage with clients to address personal issues related to their addiction and mental health, such as housing, welfare rights, medical treatment, and community development.
- Prioritize outreach work to identify and engage relevant clients across the CHO 9 area, developing targeted interventions and strategies.
- Work collaboratively with the HSE and funded services to develop an integrated care pathway for clients.
- Design and deliver evidence-based workshops, group programmes, and aftercare initiatives tailored to the needs of the client group.
- Establish connections with statutory, community, and voluntary agencies to enhance service provision and support networks.
- Identify gaps in service delivery and develop effective responses in conjunction with relevant agencies.
- Foster relationships with alcohol users, their families, and the wider community through community development and networking activities.
- Support trainee and volunteer staff in their roles, as needed.
- Maintain awareness of and adhere to all operational policies and procedures.
- Evidence of experience in addiction treatment and support services, preferably in a clinical or therapeutic setting.
- Ability to provide expert guidance and support to clients, colleagues, and other stakeholders.
- Strong communication and interpersonal skills, enabling effective collaboration and partnership building.
- Capacity to adapt to changing circumstances, priorities, and demands within the role.
- Demonstrated understanding of and commitment to promoting recovery-oriented practice and empowerment.
